Personal Choice® PPO Plans

Personal Choice PPO plans give you the ultimate in flexibility with the freedom to see any doctor without referrals, and in-network coverage available across the country.

Personal Choice® EPO Plans

Personal Choice EPO plans give you flexibility to choose any doctor or hospital in the Personal Choice® network. However, like an HMO, it doesn’t offer out-of-network coverage.

Personal Choice EPO Plans highlights:

  • In-network coverage across the country through BlueCard® PPO
  • No out-of-network coverage, except for emergency services
  • You don’t need to choose a primary care physician, or family doctor, and you never need referrals.
  • EPO Platinum plan includes the full FutureScripts pharmacy network, while the EPO Bronze Basic plan uses the Preferred Pharmacy network.
  • EPO Bronze Basic also includes the Mandatory Generic benefit
